I got the flashing D while on a road trip in North Dakota. I stopped by a local AutoZone and they were able to pull the code (P1740) for me and it was for a failed 4th gear switch. I should mentioned, the first time it happened, the van was sluggish, I turned it off and restarted it and it seemed better but not perfect.
